How to make a claim
We understand that making a claim can be both difficult and stressful during an already emotional time. That’s why our claims process is simple and straightforward. Follow the steps below and we will aim to pay your claim without any unnecessary delay.
Step 1. Contact us:
Your beneficiary should notify FiftyUp that a claim will be made. Simply call us at 1-888-441-45581-888-441-4558 or email us at fiftyupclaims@teacherslife.com and we will help you get the process started.
Step 2. Complete the required claim form and provide:
- proof of the insured’s death or terminal illness diagnosis
- the death certificate
- Attending Physician Statement to be completed by the insured’s physician, or Specialist Report if diagnosed with terminal illness
- a copy of a valid and current Canadian government issued photo ID to confirm insured’s identity
- a copy of a valid and current Canadian government issued photo ID to confirm beneficiary’s identity
- the most recent 5 years of medical history
- copy of Void Cheque
Depending on your claim, we may also require further information and documentation, including but not limited to, medical treatment records, a completed coroner’s report, Power of Attorney, or a probated will (if one has been issued), as may be required by applicable laws.

Once you have initiated a claim please contact Teachers Life on 1-888-441-45581-888-441-4558 for any further queries related to your claim.
Step 4. Claim Review
For a claim to be fully assessed, we require the completed claim form and supporting documents, as well as any additional information we may request.
Approved claims are generally paid within 2-5 business days of claim approval.
If we are unable to pay your claim, we will explain why, and you will have the opportunity to present additional information to support your claim.
